Overview
">As a skilled professional, you will be responsible for managing the care of clients in their homes, as ordered by physicians. You will work with the Visiting Nurse Association of Southeastern Connecticut to develop and implement individualized care plans that meet the unique needs of each client.
Responsibilities:
">- Develop and implement comprehensive care plans that address the physical, emotional, and social needs of clients.
- Provide direct patient care, including preventative, therapeutic, and rehabilitative nursing interventions, as ordered by physicians.
- Supervise and coordinate home health aide services, ensuring that clients receive high-quality care in the comfort of their own homes.
- Evaluate client progress, notify physicians and other healthcare professionals of changes in condition, and revise care plans as necessary.
- Identify opportunities for referrals to other health or community services and facilitate connections as appropriate.
- Promote a safe environment for clients, including addressing potential safety risks and implementing strategies to mitigate them.
Qualifications
">To be successful in this role, you will need a strong background in medical-surgical experience, preferably within the past 5 years. A Bachelor's degree in Nursing (B.S.N.) and current licensure as a Registered Nurse (R.N.) in Connecticut are also required.
