• Collaboration: Work collaboratively with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
• Educational Support: Provide educational support to patients and families to promote healthy behaviors and self-care.

Requirements

To be successful in this role, you must have:

• A current license as a Registered Nurse in the State of Connecticut.
• Graduation from an accredited school of nursing.
• Current BCLS certification or completion within one month of hire.
