Patient Care Coordinator
Yale New Haven Health seeks a detail-oriented and organized Patient Care Coordinator to assist healthcare providers in delivering patient care.
Key Responsibilities:
- Manage appointment scheduling, chart management, and ensure efficient communication between patients and the healthcare team.
- Perform clerical responsibilities, maintain patient confidentiality, and provide information to help patients navigate the practice's services.
- Foster a positive patient experience and support the overall mission and values of the healthcare organization.
Job Requirements:
- Assist physicians/providers with patient care, rooming patients, clerical, environmental and organizational tasks.
- Identify self to patient/family and explain the roles of each member of the care delivery team.
- Send/receive patient medical records, abstract new patient records into EPIC, and set up instruments and equipment according to department protocol.
- May assist and/or perform chaperone duties for patient medical procedures as needed.
Education and Experience:
- Graduation from an accredited medical assistant certificate program is required.
- Certification/Registration (CCMA/CMA/AAMA/RMA) is preferred.
- A minimum of 1 year private practice medical assistant experience is preferred in a primary care or specialist office.
Preferred Qualifications:
- Medical Assisting Certification is preferred.
- EMR experience is preferred and working knowledge of Microsoft office applications is required.
- Bilingual preference but not required.
