The Stroke Nurse Navigator Team Leader facilitates the multidisciplinary clinical program through the skills of advanced leadership resulting in exceptional patient care and clinical outcomes. The Stroke Nurse Navigator Team Leader oversees program development by championing the Navigator mission to improve patient preparedness for treatment through education and psychosocial suport while also facilitating interaction between patients, family members, their physician(s) and referrals as appropriate. The Stroke Nurse Navigator Team Leader assists in the development of appropriate educational materials and program monitoring to promote coordination of care and patient satisfaction. The Stroke Nurse Navigator Team Leader serves as the lead Navigator and liaison with the clinical and leadership team as respects program development, expansion and successful achievement of programmatic goals.

Responsibilities
- 1. Assists with all clinical and administrative aspects, initiatives and responsibilities associated with the Stroke program. Is self-directed and innovative in clinical practice. Ensures that the resources, processes of delivering care, and demonstrating the foresight to anticipate problems and solutions result in favorable outcomes.Creates and addresses all scheduling needs for the Navigators that practice across both campuses.
- 1.1 Assists physician leader and Director with ongoing management of the Stroke Program.
- 2. Provides support to patient caregivers through guidance, direction and support in both the inpatient and outpatient settings. Evaluates patients pre-admission health status to plan for discharge needs. Serves as a liaison for all EPIC-related templates, consult orders and documentation for the Navigators
- 2.1 Assists patients and families in understanding their diagnosis, treatment options and available resources.
- 3. Maintains and prepares for compliance readiness related to ongoing Joint Commission and state certificaton, or recertification, of Primary and Comprehensive Stroke Centers. Takes charge of the data abstraction, aggregation of data and action plans as it relates to the patient and family experience; provides oversight to the navigator staff for work related to follow-up phone calls and patient satisfaction information.
- 3.1 Maintains knowledge of Joint Commission certification requirements.
- 4. Assists physician and administrative leaders with Stroke Center quality improvement process. Serves as the ad-hoc representative of the Navigators on the Safety and Quality Committee. Supports the efforts of the Navigator staff to create unique approaches to individualizing patient care.
- 4.1 Participates in department specific quality improvement activity relevant to program and role.
- 5. Collaborates with physicians, nurses and staff to assess educational needs of patients, families and community. Ensures that projects related to patient and family education materials are culturally sensitive and are offered at the appropriate literacy level for the diverse patient population. Demonstrates the ability to orchestrate group projects by promoting collaboration and preventing communication breakdown among staff. Creates and supports a caring environment in the clinical and administrative settings that fosters collegiality and trust - that ultimately results in patient and family confidence
- 5.1 Assesses needs and collaborates with physicians and clinical staff to develop and provide ongoing stroke education.
- 6. Leadership: Leads the Navigator team by example and demonstrates both clinical and behavioral excellent to all members of the multi-disciplinary team. Demonstrates ongoing quest for knowledge in both the clinical practice arena and in the stroke systems of care model at the Comprehensive Stroke Center - and shares this practice across the health system
- 6.1 As requested, assists Stroke Program Manager with data abstraction and statistics.
Qualifications
EDUCATION
Bachelor's Degree in Nursing required; Master's degree in Nursing or Healthcare preferred.
EXPERIENCE
Three (3) to five (5) years experience in acute care or healthcare management required. Experience in a combination of the following is essential: clinical care, clinical process improvement, outcomes management, case management and physician relations. Knowledge and understanding of standards related to the Joint Commission certification process and "Get With The Guidelines-Stroke" preferred. Expertise in scheduling, developing aggregate reports through EPIC. Excellent oral and written communication skills. Serves as the lead liaison related to all marketing and community events.

Candidate must be able to thrive in a fast paced, complex academic medical environment where the Stroke Nurse Navigator Team Leader works as a valued member of the multidisciplinary team. Background in neuroscience/stroke care and rehabilitation preferred. Must possess understanding of clinical processes, organizational dynamics and process improvement. Must be self motivated, goal oriented and able to work independently. Must have the ability to develop relationships and work collaboratively and effectively in partnership with patients, families, physicians, clinical staff, hospital departments, community and outside organizations. Must be able to utilize the information systems and technology that translates care practices with health care outcomes. Must have excellent assessment, communication and organizational skills with the ability to manage and follow through on multiple priorities. Excellent critical thinking, analysis and assessment skills for successful process improvement planning and monitoring. Proficient in computer and data management. Identifies opportunities to engage his/her colleagues with innovative and high quality care initiatives that impact patient care, provider practice and patient / family experiences
Additional Information
Min 3 years nursing experience, preferably in Emergency Medicine, Neurosciences or Rehabilitation
BLS required - SCRN within two years of hire
