Position Focus :
The Mental Health Care Manager & Sexual Harassment and Assault Response & Education (SHARE) Clinician will provide culturally competent, trauma-informed assistance and treatment to Yale community members of diverse backgrounds. The Mental Health Care Manager role supports the operations of the Mental Health Service Line by providing clinical care management services. The SHARE Center Clinician provides direct clinical services to members of the University community impacted by sexual misconduct. This role will support 2 different departments within the Yale Health Mental Service Line, to ensure excellent care is received by members of the Yale community.
Mental Health Care Manager : Provides clinical care management for students, staff and faculty within the Yale community requiring assistance accessing all levels of mental health services. Monitors screening outcomes related to mental health and social drivers of health, to support appropriate referrals & interventions when indicated. Coordinates with the Yale Health contracted behavioral health provider network to resolve patient issues. Authorizes referrals and communicates with internal and external business partners as needed to ensure coverage of appropriate services. Functions as the primary contact for patients with regard to mental health service coordination, when issues arise. May coordinate with external mental health facilities and / or providers to identify and facilitate higher level of care options, support aftercare planning needs and ensure continuity of care across the continuum of mental health services.
SHARE Clinician : Act as first point of contact for Yale community members who have experienced sexual misconduct (sexual assault, sexual harassment, intimate partner violence, and / or stalking). Provide individual therapy and supportive counseling to Yale community members to process their experiences related to sexual misconduct. Participate in rotating schedule of SHARE’s 24 / 7 on-call phone coverage (expected 1 out of every 5 weeks). Inform community members of on-campus and off-campus resources. Interact with internal campus contacts such as medical / clinical staff and University administrators to discuss treatment and action plans.
This position will provide cross coverage and other duties as assigned.

Essential Duties
1. Plans, coordinates and implements individual, group, and couples’ therapy for students in University outpatient clinic. 2. Conducts patient evaluations and assesses patients’ acceptability for psychotherapy programs and develops initial disposition planning. 3. Establishes positive relationships with patients in order to administer the appropriate treatment. 4. Manages and provides therapy for complex cases involving patients with significant mental health concerns. 5. Provides mental health outreach to Yale community. 6. Develops materials for educational programs and seminars for professional staff. 7. Provides family meetings as appropriate. 8. Maintains patients’ charts and reports. 9. Participates in mental health 24-hour emergency coverage. 10. Provides educational and consultation services to undergraduate residential colleges and professional schools. 11. Interacts with internal contacts such as patients, patients’ families, clinicians, medical staff, and administrators regarding treatment of individual patients and implementation of clinical programs. 12. Interacts with external contacts such as clinicians in private practice, mental health institutions, university deans, hospital psychiatrists and clinicians and hospital administrators. 13. Provides clinical supervision to trainees as well as staff members. 14. Performs administrative duties as directed by the Chief of Mental health and Counseling. 15. May perform other duties as assigned.
Required Education and Experience
Master’s Degree in Social Work and three years providing clinical care post licensure.

Preferred Education, Experience and Skills :
Five years providing clinical care post licensure. Proven experience working in an outpatient behavioral health clinic, inpatient psychiatric hospital, or providing clinical care management in a medical setting. Proven experience working with EPIC is preferred.
Required Licenses or Certifications
State of Connecticut Clinical Social Worker License or Connecticut Clinical Social Worker license eligible.
