Key Responsibilities:
- Provide comprehensive care to senior patients in collaboration with a multidisciplinary team.
- Develop and implement personalized care plans using our technology platform.
- Work closely with our medical concierge team for streamlined care coordination.
- Ensure efficient communication between specialists and primary care providers.
- Participate in continuous quality improvement initiatives to enhance patient outcomes.
- Active Nurse Practitioner (NP) license and board certification.
- Experience in Geriatrics, Primary Care, or Psychiatry preferred.
