POSITION TITLE: Homelessness Prevention Specialist
RESPONSIBLE TO: Housing Stabilization Program Director / Homelessness Prevention Coordinator
RESPONSIBLE FOR:
This is a full-time, non-exempt position dedicated to delivering critical homelessness prevention, shelter diversion, and rapid re-housing services to Yamhill County community members facing the imminent threat of homelessness, those actively seeking shelter services, and those currently experiencing homelessness. This role entails conducting in-depth assessments of their circumstances, offering a comprehensive range of available services, managing intake and assessment processes, and providing light case management throughout the application process’ to ensure the stabilization of individuals and families in secure housing.
ESSENTIAL DUTIES:
Cultivate a positive team environment by actively listening, employing constructive problem-solving skills, and delivering timely responses that uphold the quality of service to clients, staff, and volunteers.
- Conduct timely and high-quality assessments, screenings, and facilitate connections to services for individuals and families facing imminent homelessness within YCAP's Homeless Prevention program.
- Develop housing stability plans, create budgets, and provide light aftercare services to households receiving assistance.
- Perform assessments, screenings, referrals, and intake procedures for community members experiencing literal homelessness, individuals, and families seeking shelter, housing, or basic necessities, with a primary goal of diverting them from the shelter system or rapidly re-housing them whenever feasible.
- Accurately and consistently input data into the HMIS Service Point system to meticulously track outcomes.
- Maintain all records, documentation, and case notes in compliance with agency and monitoring standards.
- Offer backup coverage for the Coordinated Entry system as needed.
- Collaborate with multiple funding sources to assist households in acquiring or retaining housing.
- Perform any other duties as assigned.
KNOWLEDGE, SKILLS, & ABILITIES:
- Possession of an Associate degree in a related field and/or equivalent relevant experience.
- Proficiency in computer operations, software applications, and database management.
- Strong verbal and written communication skills.
- Exceptional problem-solving abilities and a collaborative team-oriented approach.
- Demonstrated capability to maintain strict confidentiality of client and organizational information.
- Aptitude for independently managing multiple projects with a strong emphasis on meeting deadlines.
- Ability to interact effectively with diverse individuals, organizations, and a variety of situations.
- Flexibility in scheduling to accommodate the demands of the role.
- Bilingual - English/Spanish preferred
- Successful pass of drug/alcohol screening and background check.
