As a Community Health Worker, you will provide home visitation, outreach services, and educational classes specifically designed for pregnant and parenting women. The ideal candidate will be bilingual in English/Spanish.
Class specification for Community Health Worker
- Provides outreach to pregnant and parenting women within the community.
- Conducts regular home visitation to identify risk factors; provides prenatal and parenting education; reviews information related to infant/child development, immunizations, safety, etc. Connects clients with community resources as needed.
- Completes thorough documentation in a timely manner according to contract requirements.
- Assists with community prenatal or parenting education classes as requested.
- Provides support and education to nursing mothers. Links nursing mothers to community resources such as WIC as needed.
- Provides Perinatal mood disorders and domestic violence intake with education and referrals to mental health or other professionals as needed.
- Performs other job-related duties as assigned.
Education:
High school diploma or equivalent.
Experience:
Some personal experience with breastfeeding and parenting.
Additional Requirements:
Must possess a valid Arizona driver’s license.
Salary : $17 - $22
