Job Summary
We are looking for a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team, which is structured as an independent provider. The ideal candidate will deliver comprehensive care to patients of all ages, with a focus on health promotion, disease prevention, and chronic condition management. The FNP will collaborate with physicians and other healthcare professionals to provide high-quality patient care across various settings, including hospitals and outpatient clinics. The successful candidate will be self-motivated, diligent, professional, and eager to work independently as an FNP.
Responsibilities
- Conduct thorough physical examinations and health assessments for patients across the lifespan.
- Develop and implement individualized care plans based on patient needs and medical history.
- Perform triage duties to prioritize patient care effectively in fast-paced environments.
- Manage acute and chronic illnesses, including but not limited to gastroenterology issues and medical-surgical conditions.
- Provide care for toddlers and pediatric patients, ensuring appropriate developmental assessments are conducted.
- Utilize management techniques to coordinate patient care efficiently, especially for individuals with disabilities.
- Maintain accurate medical records using ICD-10 coding for diagnoses and treatments.
- Collaborate with multidisciplinary teams to ensure comprehensive patient management in settings such as PICU (Pediatric Intensive Care Unit).
- Educate patients and families on health maintenance, disease prevention, and treatment options.
Qualifications
- Master’s or Doctoral degree in Nursing from an accredited Family Nurse Practitioner program.
- Current, unrestricted Family Nurse Practitioner (FNP) license in [State].
- Board certification by the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
- Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred.
- Minimum of 2 years of experience as a Family Nurse Practitioner, preferably in a primary care or family practice setting.
- Experience in managing patients of all ages, including pediatrics, adults, and geriatrics.
- Experience with chronic disease management, preventive care, and health promotion.
- Strong clinical assessment, diagnostic, and decision-making skills.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to independently manage patient care while collaborating with physicians and other healthcare providers.
- Proficiency in using Electronic Health Record (EHR) systems. (knowledge of eClinicalWorks is a bonus)
- Ability to prioritize and manage multiple tasks in a fast-paced environment.
- Knowledge of evidence-based practices and current guidelines in family healthcare.
