Responsibilities :

  • Conduct assessments of patient needs and collaborate with the care team
  • Develop and evaluate personalized care plans
  • Coordinate and deliver physician-ordered services
  • Educate and counsel patients, caregivers, and families
  • Monitor patient progress and adjust care as needed
  • Participate in quality assurance and training programs

    • Requirements :

  • Current NY RN License
  • 1 year home health RN experience preferred
  • Experience in a qualified home health agency preferred
  • Strong communication and research skills

    • Schedule & Compensation :

  • Shift : Monday to Friday, 8 : 30 AM - 5 : 00 PM with weekend rotation
  • Full-Time : 40 hours / week
  • Rate : $33.66 - $67.05 per hour
