General Description:
The Case Manager is charged with the responsibility for the physical and emotional wellbeing of the residents of Youth Emergency Services, Inc. Work involves responsibility for the following: safety of the residents; cleanliness of Youth Emergency Services, Inc.; providing a good role model; offering emotional support; and creating a homelike atmosphere in the house.
Qualifications:
Must be 21 years old with a high school diploma or GED. 1-2 years working experience working with youth preferred.
Job Duties and Responsibilities:
Responsible of day-to-day client case managing
Complete all documentation and paperwork for clients
Attend all treatment plan, MDT, court hearing and any other client meeting needs
Arrange all medical appointments for clients
Collaborate with therapist, school DFS probation officer, judges, parents, and clients in creating and sustaining success for clients
Create a safe, structured, and caring environment for clients
Prepare nutritious meals
Ensure the cleanliness of the cottage
Supervise clients
Transport clients to appointments
Handle telephone calls and referrals
Plan activities for the clients
Daily documentation of client’s activities and behaviors
Work as a team member
Participate in at least one agency’s committee per year
Facilitate groups as assigned
Role model daily living expectations
Ensure the Agency’s mission, policies and procedures are followed
Follow all rules/policies of Youth Emergency Services, Inc.

1. Must be able to de-escalate potentially volatile situations
2. Must be able to effectively manage large groups
3. Advocacy for youth and families
4. Good Communication skills
5. Good time management and organization skills
6. Ability to work closely with other agency departments and outside agencies
7. Advanced computer skills
8. Knowledge of child growth and development
9. Ability to follow and successfully complete both written and oral direction
10. Ability to work with people of various personality types
11. Possess sound emotional judgement
12. Ability to change as the job changes
Job Type: Full-time
