Job Title: Registered Nurse (RN) - Home Health Field Nurse
Job Overview: We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide skilled nursing care to patients in the comfort of their own homes. Pay: Per Visit: based on visit type (Start of Care, Recertification, Discharge etc.)
Coverage Area: Brazoria County, TX (Brazoria, Pearland, Alvin, Manvel, etc)
Key Responsibilities:
- Provide direct patient care in accordance with the physician’s plan of care, including performing assessments, administering medications, and providing wound care.
- Develop, implement, and evaluate individualized care plans for patients based on their needs and progress.
- Educate patients and their families on disease processes, medication management, and self-care techniques to promote recovery and independence.
- Monitor patient progress and report any changes in condition to the physician and the interdisciplinary care team.
- Perform skilled nursing procedures such as IV therapy, catheter care, and other complex treatments as required.
- Maintain accurate, up-to-date patient records and documentation in compliance with state and federal regulations.
- Provide emotional support and counseling to patients and families.
- Coordinate with other healthcare professionals, including physical therapists, occupational therapists, and social workers, to ensure comprehensive patient care.
- Adhere to all company policies, HIPAA regulations, and safety protocols.
Qualifications:
- Current and valid RN license in the state of employment.
- Minimum of 1-2 years of nursing experience, with a preference for home health or community health experience.
- Strong clinical assessment skills and critical thinking abilities.
- Ability to work independently with excellent time-management skills.
- Ability to handle stressful situations with a calm, professional demeanor.
- Current CPR and BLS certifications required.
- Valid driver’s license and reliable transportation to visit patient homes.
- Excellent communication and interpersonal skills to work with patients, families, and healthcare teams.
Preferred Skills:
- Home Health experience.
- Familiarity with electronic medical records (EMR) systems. (Kinnser)
- Knowledge of home health regulations and compliance.
Physical Requirements:
- Ability to lift up to 25 lbs.
- Ability to stand, walk, bend, and squat for prolonged periods.
- Ability to travel between patient homes within the assigned area.
