Position Summary:
Facilitate delivery of YMCA’s Evidence Based Health Interventions (EBHI) by providing support and guidance to program participants, and maintain fidelity to the curriculum. This position will perform work in the community and at a YMCA facility.
Essential functions
1. Deliver YUSA EBHI curriculum to class participants in effective, meaningful and compelling ways.
2. Prepare for each session (i.e. review session contact, develop facilitation outline, review notes, and participant trackers, conduct participant reminder calls/emails/ text messages).
3. Cultivate participant commitment and support retention of participants
4. Create a safe, judgment-free, and encouraging environment that is friendly and non- competitive for participants.
5. Foster relationships with and between participants.
6. Support and encourage goal setting on a weekly basis and/or during each session.
7. Engage participants with YMCA staff & programs to connect with physical activity & other educational opportunities.
8. Travel to community locations for program delivery.
9. Follow up with participants outside of class as needed (i.e. if they are unable to attend, provide an in-person make-up session opportunity).
10. Be accessible to participants before and after sessions to answer questions and provide follow.
11. Record weekly measurements and data for each participant (attendance, body weight*, physical activity minutes*, blood pressure*, etc.). Enters data electronically within 24 hours of the session in the appropriate data base system.
12. Adhere to YMCAs HIPAA policies and procedures & ensure participants’ protected health information is secure at all times.
13. Participate in quality assurance opportunities to support fidelity to the curriculum & to meet performance standards.
14. Conduct program evaluations at appropriate times.
15. Participate in team discussions (meetings, emails or conference calls) and professional development opportunities, offer the team feedback, ideas and tools for facilitation. Remain engaged at all times.
16. Connect with program coordinator or director on a regular basis to update on program status and progress.
17. Establish relationships with public health, physician, health care providers and payer communities and other referral networks to enhance awareness of and referrals to the program.
18. Represent the YMCA at health and wellness fairs.
19. Provide healthy living support to a branch needing assistance.

Qualifications:
· Bachelor’s Degree in a health related field or in counseling preferred.
· Knowledge of basic health, nutrition, and fitness is preferred .
· Committed to and supportive of the Y mission with an emphasis on support to health seekers.
· Strong communication, organization, & relationship-building skills; the ability to build community within a group; good attention to detail; and empathy for health seekers.
· Flexible to work with people from different backgrounds and walks of life and in a variety of settings.
· Cultural competency and understanding of behavior change are required.
· Familiarity and proficiency with motivational interviewing; certification preferred
· Strong attention to detail, should be comfortable with data collection, & possess strong computer skills
· Must have flexible schedule to meet the demand of the community and corporations.
· Willingness to travel throughout the YMCA of Florida’s First Coast’s 5 county service area including: Baker, Nassau, Duval, Clay, and St. Johns Counties.
Physical Demands:
· Acceptable eyesight (with or without correction).
· Acceptable hearing (with or without correction).
· Ability to communicate both orally and in writing.
· Ability to lift up to 50 lbs.
