POSITION SUMMARY:
Provide individual and group counseling and case management services to individuals with Substance Use Disorders (SUD) to help them achieve sustained remission, restore and increase healthy social connections, obtain stable income and permanent housing, and reduce their risk of homelessness.
ESSENTIAL FUNCTIONS:
- Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders.
- Manage a caseload of program participants, providing individual counseling and case management.
- Facilitate outpatient and intensive outpatient group counseling sessions.
- Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record.
- Establish and maintain collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery, and other needs.
- Participate in daily team meetings, quality assurance meetings, in-service trainings, and coordination meetings.
- Coordinate with the Clinical Director on all critical clinical issues.
- Model key attributes: Welcoming, Genuine, Hopeful, Nurturing, and Determined.
- Perform other duties as assigned.
- Case Counselor I: Requires a CDCA II (non-preliminary), LCDCII, or SWA with 1 year of experience.
- Case Counselor II: Requires an LCDCIII, LSW, or LPC, a Bachelor's Degree, and 2 years of experience.
- Case Counselor III: Requires an independent license or an LCDCIII, LSW, or LPC with a Master's degree (preferred) or enrollment in a Master's program with 3 years of experience.
- CPR and First Aid certification and Safety training required within 30 days of hire.
- Must have successful experience facilitating SUD group counseling.
- Extensive knowledge of Substance Use Disorders and issues affecting the homeless population.
- Basic computer skills, including Microsoft Office applications.
- Knowledge of OHMAS, CARF, and VA program compliance standards preferred.
- Bilingual (Spanish/English) preferred but not required.
- Commitment to the mission and values of the YMCA.
- Ability to work effectively with individuals from diverse backgrounds.
- Willingness to serve as a mandated reporter for child abuse and neglect.
- Must be able to express or exchange ideas clearly through speech.
- Must be able to hear and perceive sounds at normal speaking levels.
- Frequent standing, walking, reaching, stooping, kneeling, and bending.
- Sedentary work, requiring minimal physical exertion but prolonged sitting.
- Close visual acuity required for reading, computer use, and detailed tasks.
- Work is primarily conducted in an office setting with no substantial exposure to adverse environmental conditions.
