A DAY IN THE LIFE OF A CASE MANAGER
As a Case Manager for our Erie County Re-Entry Services Support Alliance (ECRSSA) program, you are responsible for the development and coordination of service support plans for transitioning clients and community based clients who are identified through the Group Violence Reduction Strategy (GVRS) and Reentry Support Services Alliance. Duties require working one-on-one with individuals to identify needs and connecting those individuals with the appropriate services to assure barriers are eliminated that would prevent follow through of service. Responsibilities include coordinating linkages and referrals to community providers for employment opportunities, education/training, housing, transportation, treatment services, mentoring and other comprehensive supports and services.
You are someone who has existing or can build relationships with the community and can leverage those contacts to support the program as you work directly with transitioning clients and local service providers to develop a plan for reintegration from incarceration into the Erie community. You are also conscientious about keeping records up to date and accurate and ensuring compliance with YMCA standards and government regulations. As someone who loves your job, you happily attend and participate in all trainings and program activities.
