Position: Family Case Manager Newcomer Salary: $50,000-53,000
About the Job:
- Assist immigrant individuals and families adjust to life in a new country, secure basic resources and arrange support for legal matters, such as documentation.
- Assessing legal needs and connecting clients with legal services.
- Served as a liaison between ICE, USCIS and Immigration court.
- Check case status to ensure clients court dates and ICE check points are up today.
- Receives and assesses client referrals: interviews clients to determine client need for assistance.
- Serves as a liaison to develop, monitor, and adjusts family service plan and individualized client services: Working under the general supervision of a program supervisor, independently arranges the sequence of work, obtains necessary information and data, selects appropriate methods and procedures, and varies these as necessary to address different case situations.
- Provides general client services and case management: offers significant support and guidance, as well as service assistance, which directly impact the social, physical and/or economic well-being of the clients served and, at times, their families/significant others.
- Accepts referrals for social services and provides feedback on referrals
- Assists clients to assess their situation, define their problems, and identify acceptable courses of action by availing themselves of recommended community services.
- Completes initial intakes/assessments
- Analyze the subjects or issues involved in each assignment or case to develop family service plans for client’s family
- Determine intervention strategy and appropriate actions utilizing client’s individual needs
- Connect clients to outside service providers and resources
- Provides case management and monitoring services to clients
- Makes follow-through contacts with clients and other agencies to ensure that needed services have been provided
- Prepares and updates case records/client status reports
- Ensures recordkeeping-intakes/family needs assessments; family service plans, contact notes, electronic database entry
- Keeps abreast of changes to and the issuance of new rules, regulations, policies and procedures pertaining to program eligibility and service delivery
- Reviews programmatic and personal documents, creates and maintains case notes and records, drafts brief memos and letters, and performs related functions
- Bachelor’s degree in human services. Comparable experience may be considered as equivalent for satisfying educational requirement(s).
- 1-3 years’ experience
- Experience in working with diverse populations; and skilled at collaborating with multiple agencies
