As a Full-Time Case Manager at the YMCA of Reading & Berks County, you will dive into a role that's both dynamic and impactful! You'll kick things off by reviewing referrals and interviewing potential clients, ensuring they receive a warm welcome with house rules orientation. Your organizational skills will shine as you maintain detailed case management files, aligning with Bureau of Drug and Alcohol Program guidelines. You'll be the cornerstone in connecting clients to essential outpatient treatment and conducting thorough assessments of their health, education, and vocational needs within a week of admission. By crafting individualized service plans in collaboration with clients, you'll aid their journey to recovery.
Regular one-on-one meetings and open communication with treatment counselors will enrich your clients' experiences, while you supervise case manager assistants to uphold our core values. You'll also run house meetings, coordinate billing, and ensure compliance with all ethical and safety standards-making every day an exciting adventure!

A Bachelor's degree with at least one year of case management or counseling experience (or an equivalent combination) sets the foundation for your success. CAC and CAAP certifications are preferred, while maintaining sobriety is a must for self-identified recovering addicts. Being punctual and possessing a valid PA driver's license ensures you're always ready to roll. Knowledge and skills required for the position are :
- 1. Bachelor's degree with 1 year of case management and / or counseling experience OR Associates Degree and 2 years of case management and / or counseling experience OR a high School Diploma with 3 years of case management and / or counseling experience OR a any equivalent combination of education and experience.
- CAC and CAAP preferred.
- If self-diagnosed as recovering addict must remain clean and sober; maintain sobriety and active program recovery. Ability to communicate positively and professionally using the English Language; both spoken and written.
- Ability to demonstrate good decision making skills, intelligence, flexibility, emotional maturity, a positive outlook, respect, and a professional attitude towards all clients and staff.
- Ability to make decisions about a client on a case by case basis.
- Ability to communicate positively and demonstrate enthusiasm.
- Have active PA driver's license. Be on time for interviews, appointments, and meetings.
- Ability to treat all clients (regardless of the circumstance) with dignity and respect.
- Ability to maintain a positive, caring, and empathetic attitude.
- Ability to listen and be present.
