ESSENTIAL FUNCTIONS:
- Client Support: Assist clients in navigating resources, including housing, healthcare, education, and employment services.
- Resource Coordination: Establish and maintain partnerships with local agencies, organizations, and service providers to facilitate access to services.
- Outreach and Engagement: Conduct outreach activities to identify and engage potential clients, building trusting relationships within the community.
- Documentation and Reporting: Maintain accurate records of client interactions, services provided, and outcomes; prepare reports for management.
- Workshops and Training: Organize and facilitate workshops and training sessions to empower clients with skills and knowledge.
- Transportation Coordination: Assist with coordination of transportation services for tenants.
- Policy Support: Support YMCA procedures and policies.

- Bachelor's degree in Social Work, Psychology, Human Services, or a related field (or equivalent experience).
- Experience in case management or supportive services is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of community resources and services available for clients.
- Proficient in Microsoft Office Suite and case management software.
- While performing the duties of this job, the employee is regularly required to use a computer for extended periods of time and be able to communicate using a computer and phone.
- The employee frequently is required to sit and reach, and must be able to move around the work environment.
- The employee may have to complete a job where repetitive motions are required.
- Employee must be able to communicate effectively by speaking and listening.
- The employee must occasionally lift and/or move up to 10 pounds.
