Essential Job Functions & Responsibilities:
- Providing direct patient care under the supervision of the Hospice Clinical Manager.
- Makes home visits as necessary. Works cooperatively with patients, families, nursing staff, physicians and ancillary service providers.
- Conducts patient triage from patients/families.
Job Qualifications:
- A current license in the State of California as a Licensed Vocational Nurse.
- Ability to communicate effectively verbally and in writing in English is required. Spanish language skills are also encouraged.
- One (1) year of hospice experience desired; two (2) years SNF/home care experience.
- Knowledgeable about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards.
Pay range: $32-$35/hour
