We have a new opening for an RN Care Manager. This is a full time position working in Kittery Family Medicine and is located in Kittery, Maine.
The RN Care Manager works with the practice team to provide proactive, comprehensive care management within the patient-centered medical home model. Together with clinical and administrative leadership and other members of the Practice Team, will be responsible for attaining and maintaining Patient Centered Medical Home NCQA Recognition for the practice.
Job Responsibilities include the following:
- Assist in creating and maintaining care management systems to prospectively identify and track patients with chronic disease, or who are at risk to chronic disease and/or patients with diagnoses that put them at high risk for hospital admission.
- Work with the practice team to develop a care plan using protocol-driven care management.
- Proactively engage patients in their care with strategies to improve self-management.
- Manage the care transition of patients to and from the ED, hospital and other institutions.
- Attest to the maximum number of MaineCare patient on the Health Home portal on a monthly basis.
- Coordinate Care Management with the contracted Community CareTeam.
- Assist with developing practice policies that meet NCQA Patient Centered Medical Home standards.
In order for us to continue to provide exceptional patient/customer experiences, we need you to have the following:
- Minimum of 3 years in primary care practice
- High level of clinical care skills including patient communication and education skills
- BSN
- Able to problem solve
- Excellent communication/process skills
- Organized
- Active RN License.
