Project Overview
The Phoenix Project provides relentless outreach, one-on-one coaching and multiyear support to participants in need of trauma informed care living in the four HOPE SF communities (Hunters View, Alice Griffith, Sunnydale and Potrero Terrace & Annex) and two (2) additional Rental Assistance Demonstration (RAD) sites (Oakdale and Harbor). The Phoenix Project utilizes a bio-psychosocial-cultural-spiritual (BPSS) framework to holistically address individuals’ needs and facilitate trust and development. Leveraging a collaborative of eight (8) community-based partners , the Phoenix Project provides a holistic approach to service delivery, designed to work with young people who do not feel safe and valued in traditional programming and institutions. Phoenix Project partners provide life coaches/peer advocates, educational/employment services, behavioral/physical health services, peace promotion, and barrier remediation services.
POSITION SUMMARY
This is a temporary full-time position from May 1, 2025 – September 30, 2025. This position is in person Monday – Friday 10:00am –6:00pm. The Life Coach will utilize evidence-based practices and combined experiences working with TAY (transitional age youth 14-24) population to effectively serve participants disconnected from resources. Life Coach must understand the mission of the Phoenix Project is to support TAY living in HOPE SF/RAD sites with the goal of improving graduation rates, employment/ career opportunities, and assistance toward stable housing.
The Life Coach engages disconnected youth in the Phoenix Project model. In addition, the Life Coach must understand the fragmented, traumatizing and inequitable systems that often constrain opportunity for youth residing in public housing. Furthermore, must successfully guide TAY clients through such barriers by creating Life Maps. Most importantly the Life Coach must create intentional and authentic transformational relationships necessary for the Phoenix Project model to work, understanding the historical trauma, mistrust, and isolation many youth living in HOPE SF/RAD sites may experience.
RESPONSIBILITIES:
- Responsible for the maintenance of consistent program and participant documentation, including an updated log of all clients and constituents, the nature of those contacts, and recommendations for follow-up as needed.
- Drives participants to and from home and other locations in agency van and/or personal vehicle when necessary.
- Possible rotating Saturday work days depending on TAY youth needs and Directors Approval
- Service all HOPE SF/RAD Sites (Potrero Hill, Hunters View, Alice Griffith, Sunnydale, Oakdale, Harbor Rd) for outreach, intake and building relationships.
- Supervise their subordinate staff/ Youth Development Advocate (YDA’s)
- Create a schedule for each YDA in alignment with Phoenix clients
- Forge relationships with key stakeholders in each HOPE SF/RAD Site to extend the reach of the Phoenix Project
- Submit two highlights per month representing clients
- Coordinate daily meeting at 5pm with their YDA staff to provide updates and goals and objectives for phoenix clients for the following day.
- Provide treatment plan (Individual Mobility Road Maps) for each assigned TAY participant
- Identify educational, social, employment and health needs of TAY participants in program while providing trauma informed care
- Must implement in-depth assessments for TAY youth geared toward measuring immediate support and well being
- Will perform Motivational Interviewing, Strength Based Approach practices geared toward building transformational relationships with TAY youth
- Plan workshops in support of TAY that are in direct alignment with their Individual Mobility Roadmaps (IMR)
- Demonstrate sufficient knowledge of community resources and relevant intervention strategies TAY youth may need living in areas with high levels of various forms of trauma
- Comfortable performing trauma, resilience, wellness and ethnic identity measurements to TAY clients in a culturally competent manner.
WHAT YOU WILL BRING
A successful candidate will possess:
- B.A./B.S. in Psychology, Sociology, Social Work, Urban Planning, or Nonprofit major or minimum of 5 years experience working with youth and young adults in trauma impacted areas.
- Demonstrate competency with using Salesforce, data entry, Microsoft Office Suite, Google platform, Zoom and other technology platforms.
- Valid California Driver’s license with proof of insurance.
- Excellent cross-cultural communication skills and the ability and willingness to work with economically and culturally diverse clientele from various professional and ethnic backgrounds.
- Excellent interpersonal and communication skills, written communications, patience, diplomacy, and high level of customer service skills.
- Ability to think critically, problem solve and be resourceful.
- Embraces cultural differences and is willing to foster diversity, equity and inclusion.
- Experience supervising team members and has the capacity to leverage best practices such as motivational interviewing and coaching.
- Self-motivated individual who has a passion to ensure that program goals, objectives and tasks are completed in a timely fashion and exceeds standards.
- An efficient multi-tasker who demonstrates effective time management and the capacity to meet competing deadlines.
- Must have proven ability to work both independently and as part of a team.
- Demonstrate effective written and verbal communication skills.

COMPENSATION: $30- $32 hourly
