Bilingual in Spanish/English required. Daytime (10:30-12:30 pm) and/or evening (5:30-7:30 pm).
POSITION SUMMARY:
Under the direction of the Senior Manager Health and Wellbeing, Diabetes Prevention Program, the Lifestyle Coach facilitates delivery of the YMCAs Diabetes Prevention Program, provides support and guidance to program participants, and maintains fidelity to the curriculum. The Lifestyle Coach will deliver professional customer service and relationship building skills to create a welcoming environment in the program. The incumbent will also exhibit the core values of caring, honesty, respect and responsibility in all aspects of their work with the YMCA.

ESSENTIAL FUNCTIONS:
- Deliver curriculum to program participants in effective, meaningful, and compelling ways.
- Deliver curriculum in both English and Spanish as needed per cohort.
- Flexibility to deliver daytime, evening or weekend classes.
- Encourage group participation and interaction through open-ended questions and other facilitation skills
- Cultivate participant commitment and support retention of participants
- Create a safe, judgment-free, and encouraging environment that is friendly and non-competitive for participants
- Foster relationships with and between participants
- Make learning a shared responsibility for the group
- Support and encourage participant goal setting at each session
- Travel to community locations for program delivery (ability to deliver at multiple Metropolitan Denver YMCA locations preferred)
- Flexibility to deliver daytime, evening or weekend classes
- Prepare for each new session (e.g. review session content, develop facilitation outline, review notes and participant food and activity trackers, conduct participant reminder call/send participant reminder email
- Weigh or collect weight information from participants at each session
- Be accessible to participants before and after sessions to answer questions and provide follow-up
- Follow-up with participants outside of class as needed
- Schedule and conduct in-person make-up sessions
- Record weekly data for each participant (attendance, weight, physical activity minutes, food tracker completion) in Welld Health database within 48 hours of session
- Adhere to the YMCAs HIPAA policies and procedures and ensure participants’ protected health information is secure
- Required to participate in quality assurance opportunities to support fidelity to the curriculum and to meet performance standards
- Arrive for class on time and dressed appropriately wearing nametag during program
- Required to participate in all required Lifestyle Coach team discussions (meetings, emails, or conference calls) and professional development opportunities
- Connect with Senior Manager. Health and Wellbeing on a regular basis to update on program status and progress and as per any additional expectations outlined.
- Follow and enforce all YMCA procedures and policies, including personnel guidelines, safety guidelines, facility access procedures, and membership policies. Carry out emergency plans as necessary.
- Attend and remain current on all mandatory trainings.
- Be knowledgeable and supportive of the YMCA annual support campaign.
- Any other duties as assigned
WORK ENVIRONMENT & PHYSICAL DEMANDS:
- While performing the duties of the job, the employee is regularly required to stand, walk, sit; use hands to finger, handle or feel objects; reach with hands and arms; climb, balance, stoop, kneel crouch, crawl; talk and hear.
- The employee must regularly lift up to 10 pounds.
- The vision abilities required by the job include: close, distant, peripheral, depth perception and the ability to adjust focus.
- The employee is occasionally exposed to a moderate level and occasional high noise levels. The employee may occasionally be subject to outdoor weather conditions.
Qualifications & Benefits
QUALIFICATIONS
- Bilingual – English and Spanish, required
- Current certification in CPR and First Aid is preferred
- Knowledge of basic health, nutrition and fitness is preferred
- Committed to and supportive of the Y mission with an emphasis on support to health seekers.
- Must have strong communication and relationship building skills and the ability to build community within a group and have empathy for health seekers
- Must be flexible to work with a wide variety of people. Experience in working with the older adult population preferred.
- Familiarity and proficiency with Listen First or motivational interviewing highly encouraged.
- CDC approved, DPP Lifestyle Coach training certification or will train after hire
