POSITION SUMMARY: The PSRS position is an integral part of the direct care staff in ensuring client safety while clients progress towards rehabilitation goals daily. The PSRS has the responsibility of assisting in improving an individual’s process of restoration of community functioning and well-being. This position seeks to effect changes in a person's environment and in a person's ability to deal with his/her environment through coaching. These services are combined with independent living and social skills training, vocational rehabilitation, social support and network enhancement, and access to leisure activities. Major duties of this position are in the area of service plan implementation; client advocacy; therapeutic intervention with clients in crisis, service documentation and reporting; and client social, recreational and educational activities. The PSRS is to work collaboratively in order to empower clients, and sometimes on a goal of full psychosocial recovery.
Psychosocial rehabilitation services are collaborative, person directed and individualized. They focus on helping individuals develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning, and social environments of their choice. This position is under direct management/ lead supervision and has the responsibility to follow through with tasks as trained and as assigned.
Duties and Responsibilities:
1. Work in conjunction with the treatment team to provide a coordinated approach.
2. Provide interventions and services to promote the highest possible level of success – socially, self-care, increase positive coping skills – through restoration and maintenance, which can include, but not limited to, individual or group sessions, case management, planned, experiential learning experiences, tutoring, skill development through supported exercises, etc.
3. Maintain a positive and professional working relationship with other staff, other centers, residential care facilities, referral and resource persons, community persons, families, and any other individuals who advocate or provide collaborative services for the consumer.
4. Be responsible for the timely documentation of services provided including, but not limited to the case management plan, service logs, treatment reviews, or any other clinical documentation needed for quality care.
5. Provides referral, linkage and advocacy while working with the individual to gain access to appropriate community resources (i.e. needed medical, social, educational services) essential to meeting basic human needs.
6. Work in collaboration with qualified staff, teachers, other professionals and Child Welfare.
7. Attend meetings as deemed appropriate (i.e. team meetings, recovery planning meetings, supervision).
8. Maintain current case management certification through the Oklahoma Department of Mental Health and Substance Abuse Services.
9. Attend trainings on recovery, psychiatric rehabilitation, trauma and cultural competency.
10. Perform other duties/responsibilities as assigned by the Clinical Director or his/her designee.
Qualifications:
1. Completion of the curriculum requirements for a B.A. or B.S. is required.
2. Completion of training in Strengths Based Targeted Case Management II and receives certification of completion from ODMHSAS for Strengths Based Targeted Case Management II.
3. Knowledge and understanding of general psychosocial standards and medical practices associated with the treating of mental illness.
4. Valid driver’s license.
