Job Description
ICC/HFW Facilitator
Brief Job Summary:
The ICC/HWF Facilitator is responsible for coordinating monthly team meetings with the family, professional system partners, and natural supports identified by the family. Skilled ICC/HFW workers will require and utilize many clinical skills including relationship building/engagement, soliciting and empowering client voice, conflict management, facilitating group process, understanding and management of group dynamics, knowledge of clinical and related community services, and development of case plans, crisis intervention planning, and monitoring progress.

Qualifications:
- Must be a team player and have good communication and organizational skills
- Must be willing to lead meetings
- Ability to write case notes that incorporate all aspects of interactions appropriately and thoroughly
Main Functions:
- Coordinate monthly team meetings
- Participate in face-to-face meetings with the family
- Educate necessary parties regarding available community resources
- Maintain confidentiality, respect human rights and practice universal precautions in accordance with organizational policies and procedures
- Participate in supervision with the clinical director
- Attend court hearings to provide testimony and treatment recommendations
- Collaborate with other service providers to ensure clients and families receive the best level of care
Work Schedule:
- Work hours vary
- Evening hours required
Additional Requirements:
- Must have a valid driver’s license in good standing
- Must have a valid automobile insurance policy
- Successful clearance of at least three professional references
- Successful clearance of all required background and registry checks
Educational Requirements:
- Bachelor’s Degree with experience working with youth and families
Job Types: Full-time, Part-time
Pay: $20.00 - $28.00 per hour
