What are the responsibilities and job description for the Residential Coordinator position at Youth Intensive Services?
JOB SUMMARY
Responsible for the coordination of services and documentation for the adolescent population at Youth Intensive Services (YIS).
ESSENTIAL FUNCTIONS: include the following: Employee must have regular attendance/punctuality, be able to cooperate with others, and be completely honest. Other assigned duties include:
PRIMARY DUTIES AND RESPONSIBILITIES:
1. Ongoing assessment of needs.
2. Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian.
3. Facilitation of further development of daily living skills, if identified by the individual and/or parent or guardian.
4. Coordination of the ISP, including: services identified in the ISP, assistance with accessing natural support systems in the community, and linkages to formal community service/systems.
5. Symptom monitoring.
6. Coordination and/or assistance in crisis management and stabilization as needed.
7. Advocacy and outreach.
8. As appropriate to the care provided to individuals, and when appropriate, to family, education and training specific to the individual’s assessed needs, abilities and readiness to learn.
9. Mental health interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment.
10. Activities that increase the individual’s capacity to positively impact his/her own environment.
OTHER DUTIES AND RESPONSIBILITIES:
1. Contact, schedule, interview and respond to potential students eligible for treatment.
2. Present potential student to team
3. Ensure funding has been secured by guardian and collect insurance.
4. Coordinate with Case manager and team intake
5. Continuously monitor and update Credible information as student moves through treatment.
6. Attend all monthly team, staff and clinical meetings.
7. Coordinate, schedule, facilitate Aftercare for 6 months after students discharge.
8. Completing all necessary documentation for all youth in the program, including admissions, initial Treatment Plans, Behavior Plans, and monthly summaries. Maintain ongoing records of all case management contacts. Create progress summaries and share as scheduled with the referring agency.
9. Completing discharge instructions which are prepared for the discharging party at the time of the youth’s discharge from the program. Completing final discharge summaries within the proscribed period of time, and appropriately sharing the summary with the referring party. Creating after care recommendations, recreational opportunities and contacts, and clinical guidelines for placement as part of the aftercare program.
10. Ensuring all necessary billing information is provided for accurate billing in a timely fashion.
11. Responsibility to ensure that each youth file is accurate and contains all pertinent information.
12. Facilitating and recording a bi-weekly clinical staffing and for each youth, ensuring that the licensed clinical treatment provider is present at the staffing.
13. Maintaining all necessary paperwork and releases that enable youth to safely and legally participate in treatment activities. Drafting a monthly summary regarding each youth’s participation in group treatment activities.
14. Coordinating activities, to include intake paperwork, project creation and supervision, tracking of hours worked, etc.
15. Representing youth at parole hearings, community review boards, court, secure placement hearings, expulsion hearings, etc.
16. Coordinating and monitoring all emancipation activities occurring off campus, including tracking youth at work, treatment groups, recreational activities, and coordination of after care.
17. Coordinating outside agency admissions and admission interview preparation. Acting as liaison with employers, and attending weekly staffing meetings at the YIS.
18. Overseeing the youth, including approval of visits and phone contacts. Maintains approved contact lists.
19. Overseeing the medical needs of the youth, including health, dental, and vision needs. Ensures medical appointments are arranged and attended in a timely manner. Ensuring follow up care is provided, and medical information is communicated as appropriate.
20. Acting Group Living Director in the absence of the Group Living Director.
21. Other duties as assigned.
Qualifications:
1. A bachelor's degree from a regionally accredited college or university with a major in behavioral science, human services or related fields, and three years’ experience in working with children, or a combination of experience, education, and training that provide the necessary knowledge, skills and abilities is required.
2. Must pass mandatory background checks.
Physical Requirements:
- Ability to apply Safe Crisis Management when necessary.
- Ability to physically assist in lifting and carrying/moving furniture as necessary and as directed from your supervisor.
- Ability to visually and audibly assess children behavior and needs.
- Ability to walk and/or run to supervise active games with children.
Job Type: Full-time
Pay: $16.00 - $19.00 per hour
