Position Overview
The Case Manager provides comprehensive support to individuals and families in Transitional Housing (TH), Rapid Rehousing (RRH), Maternity Group Home (MGH), and Emergency Shelter programs. This role focuses on housing stability, resource navigation, goal planning, and client empowerment. The Case Manager ensures compliance with program requirements while maintaining a trauma-informed and client-centered approach.
Key Responsibilities
Client Support & Case Management
- Conduct client intakes, assessments, and individualized goal planning.
- Provide case management services to clients in TH, RRH, MGH, and Emergency Shelter programs.
- Meet with clients weekly, with at least two in-person meetings per month.
- Connect clients with appropriate resources, including housing assistance, employment, education, childcare, and healthcare.
- Assist clients in obtaining vital documents such as IDs, birth certificates, and social security cards.
- Monitor and support clients in achieving stability, addressing barriers, and adjusting goals as needed.
Housing & Shelter Support
- Assist Housing clients with housing searches and landlord engagement.
- Conduct Housing Quality Standards (HQS) inspections and ensure Fair Market Rent (FMR) compliance.
- Guide clients through lease signings and move-in processes, providing necessary household supplies.
- Ensure MGH and TH units are clean and prepared for new residents.
- Support Emergency Shelter clients in securing permanent housing, family reunification, or other placements.
Documentation & Compliance
- Maintain accurate client records in case management systems (e.g., Extended Reach, HMIS).
- Ensure case notes reflect scheduled meetings and services provided.
- Complete required intake, progress, and discharge documentation within deadlines.
- Submit weekly schedules and comply with reporting requirements for all programs.
Program Coordination & Collaboration
- Work closely with internal teams and external partners to provide comprehensive client support.
- Educate clients on available resources and thoroughly explain service options.
- Participate in program meetings, staff training, and continuous professional development.
- Monitor MGH unit cleanliness, facilitate monthly house meetings, and track child development for parenting clients.
Qualifications & Skills
Education & Experience
- Required: High School Diploma or equivalent.
- Preferred: Bachelor’s degree in Social Work, Human Services, or a related field.
- Required Experience: 2 years of experience in case management, housing programs, or youth services.
Skills & Competencies
- Strong organizational, communication, and problem-solving skills.
- Familiarity with housing programs, resource navigation, and trauma-informed care.
- Ability to use case management systems (Extended Reach, HMIS) and maintain compliance with documentation standards.
- Capacity to work independently while collaborating with a multidisciplinary team.
Work Expectations
- Case Managers must be flexible, responsive to client needs, and available for in-person meetings and on-site support.
- Compliance with program policies, deadlines, and reporting requirements is essential.
- Some evening or weekend availability may be required based on client needs.
