Key Responsibilities:
Provide Case Management services and ongoing communication, evaluations, and assessments to youth, families, and Juvenile Probation Officers during the transition stage, under the direct clinical supervision of a licensed mental health professional. Assist youth with employment/education/training options and career planning.
- Liaison with the assigned case manager and treatment team, to ensure effective transition back into the community.
- Organize the plan to include education material such as educational transcripts, GED and/or Diploma; support services such as housing and transportation; critical documents, such as a copy of birth certificate, a copy of social security card, a state issued I.D. card, employment/post-secondary education appointments such as Career Source Centers; a completed employment application, a completed resume, and healthcare transition with a focus on medical appointments and Medicare eligibility.
- Communicate with external community sources regarding family/residents on their caseload and supports family contact.
- Provide a community resource guide and connect the youth to applicable support groups such as AA/NA another community-based relapse support programs.
- Develops, delivers, and facilitates individual program plans for residents.
- Assists residents with becoming aware of strengths and needs in adjusting socially to their environment and facilitates the development of life skills for successful re-entry into the community.
- Oversees the transitional process of each client.
- Responsibilities involve facilitating the case management process which includes leading the Treatment Team, developing Treatment Plans, evaluating student needs, and ensuring the timely delivery of appropriate program services to meet these needs within the established guidelines.
- Serves as a liaison between the family/parents and the Program and between the appropriate State Agency and the Program.
Other Duties Include:
- Maintain a high standard of role model behavior for youth.
- Participate in training and development activities as required.
- Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships.
- Report any use of physical force and all unusual incidents per guidelines.
- Perform other duties as assigned or needed.
Requirements:
Education: A bachelor’s degree from an accredited university or college in the field of counseling, social work, psychology, rehabilitation, special education, or in a related human services field.
Experience: Two years ’experience working with children with serious emotional disturbances or substance abuse problems. If the candidate has a master’s degree in an appropriate field, the experience requirement is preferred, but not required. Experience with state’s Department of Juvenile Justice and group work techniques is a plus.
Skills: A sound understanding of group leadership, group dynamics, and experiential education. Records management and Medicaid processes preferred. Advanced oral and written communication skills.
Physical Demands: Must be in good physical health and emotionally stable. Must be able to move about the property. Must be able to participate in youth activities. Must be able to transport youth or staff. Must be free of communicable diseases as defined by the state and to the extent that data is attainable under the law. Must be able to pass evaluation of physical techniques and written certification exam.
Other: Must maintain an appropriate and valid state driver’s license. Must meet state criminal background check requirements.
