POSITION: Child and Family Finding Team Facilitator
QUALIFICATIONS: Master’s degree in social work, psychology, education or related field. Previous experience and knowledge of Child Welfare and Licensure processes a MUST. Sensitivity and responsiveness to the cultural differences of individuals and families in groups outside one's own.
CLASSIFICATION: Exempt, Full-Time
REPORTS TO: Director of Child Welfare
Summary: In coordination with the Family Finding and Child Welfare Director, the Family Finding Facilitator will hold Child and Family Team Meetings to establish a strong plan for permanency for clients.
I. Duties:
· Oversees Adolescent Mentor position
· Prep notes and facilitate Child and Family Team Meetings (CFTMs).
· Track when CFTMs are needed
· Holds 3-5 CFTM’s per day, ALL VIA Teams or Zoom (no in-person).
· Facilitates meetings with Caseworkers on client cases to identify needs.
· After CFTM, must write a case note
· Maintains professional boundaries:
· Treats all program participants and families equally.
· Follows agency and program procedures regarding interactions with participants and their families.
· Maintains confidentiality of participant/agency information.
· Demonstrates professional, assertive communication skills with participants, co-workers and supervisor.
· Understands and supports program goals/agency mission.
· Has read and understands policies and procedures related to position
· Establishes/develops/maintains relationships with local community resources on behalf of the community committee.
II. Other Duties
● Actively participates in YSBIV's Continuous Quality Improvement program (CQI).
