Youth Services System (YSS) is looking for a Wraparound Facilitator to join our team and help provide essential services to children and families in our West Virginia communities.
As a Wraparound Facilitator, you will be responsible for implementing Wraparound Services using the Wraparound Model, which emphasizes collaboration, coordination, and strength-building for youth and families. You will work closely with youth aged 12 to 17 who face severe emotional or behavioral challenges, helping them overcome barriers to daily functioning. Your role is crucial in creating individualized plans for children and families while ensuring their needs are met in a supportive, trauma-informed manner.

Wraparound Facilitator Required Skills/Abilities:
- Manage cases for Youth and Family Wraparound Services.
- Engage intensively with youth, their families, and the wraparound team to build strong relationships.
- Facilitate family team meetings, including scheduling and documentation.
- Provide training and guidance to family team members on the wraparound process.
- Advocate for and arrange services and supports as outlined in the wraparound plan.
- Track and assess progress towards team goals and individual client needs.
- Work collaboratively with treatment providers, support services, and natural supports to tailor services for each family.
