Job DetailsJob LocationHazel Atlas - Wheeling, WVEducation Level4 Year DegreeTravel PercentageUp to 25�scriptionJob Summary : Under supervision of the departmental manager the Case Manager II is responsible for planning, coordinating and ensuring the delivery of necessary and appropriate services for assigned clients within established timeframes. Additionally, the Case Manager II is responsible to ensure the accurate assessment of psychological test scores as well as provide detailed behavioral observations to aid in the interpretation of psychological tests. This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices.Duties and Responsibilities : Insures and monitors client eligibility by Accessing current Medicaid numberDeveloping interim care plan within 7 days of client admissionConvening client care plan within 30 days of admissionConvening review of care plan every 90 days or sooner if clients plan warrants.Insuring that all plans and initial evaluations are signed by physician or licensed psychologist. Performs or causes to occur adequate and comprehensive assessment of client needs to develop and design client care plan and then continually assesses client for further needs that require provision of services.Typical Assessments are : Social History Psychological / Psychiatric EvaluationsClinical EvaluationDrug and Alcohol AssessmentBasic Living Skills Assessment Behavior AssessmentEducational Needs AssessmentConducting face to face interviews with clients and family to develop family assessment; schedule face to face interviews with client's; caretakers to assess current needs / ongoing needs; scheduling opportunities to review client documentation, reports and case record. With client input developing and prioritizing client needs that become the basis for developing and coordinating clients Plan of Care.Convenes, chairs and facilitates client care plans within 30 days of admission. Designs care plan with any required budget subsequent to care plan meeting.Advocates for client and family needs and services with court systems, human service providers, health systems, as well as in Agency services providers. Independently resolve problems in service delivery.Assures linkage to all identified internal and external services as indicated in client Plan of Care.Monitors and evaluates client progress, reassesses client needs and develops further plans of care (every 90 days) based on evaluations. Is available and responds to crisis intervention needs of client.Develops and maintains complete, accurate and per Agency timelines, all case record documentation (Plans of Care, discharge summaries, etc.) Travels on behalf of client for client needs and services.Consults with Program designee weekly to monitor and direct their client service delivery.Attends all meetings and court hearings involving clients as assigned.Attends all required training's, in-service, as well as keeps current social work licensure. Actively participates in supervision with case management coordinator.Administer and score Psychological instruments for a population composed of children, adolescents, and adultsAssess and report client's level of cooperation and participation in the assessment processPerform other duties as assigned. Prefers 2-3-year experience with youth / young adults with mental health and substance abuse diagnosis; Must have valid driver's license and proof of automobile insurance as required by law.Required Skills / Abilities : Knowledge and experience with adolescents with mental health diagnosisKnowledge of various systems including child welfare, mental health, education, and judicialAbility to work under pressure and multitaskExperience with Electronic Medical Records SystemsAbility to work under pressure and multitaskProficient and accurate documentationGood written and oral communication skillsAbility to use Adobe and Microsoft softwareEnsures accuracy, security, and availability of informationKnowledgeable in the administration, scoring, and evaluating of testing instruments.
