JOB DESCRIPTION:

The Case Manager will be responsible for Service Planning for the Child and will be the point of contact for the child’s family and referring worker. The Case Manager is responsible for coordinating with referral sources and coordinating services for the child that will include the child’s educational, medical and behavioral health needs. The Case Manager shall be the designated official of “Reasonable and Prudent Parent standard” and make careful and sensible parental decisions that maintain the health, safety and best interests of a child while at the same time encouraging the emotional and developmental growth of the child.

JOB RESPONSIBILITIES:

Provides care planning and coordination for youth.

  • Schedule and facilitate a service planning conference within 5 days of placement.
  • Develop and review a service plan for the child that integrates services that are culturally competent and addresses language, cultural and ethnic needs. The plan will include a reintegration plan and crisis plan.
  • Life skills assessments and planning will be completed for youth
  • Ensures that all medical, dental, and behavioral health appointments are scheduled and attended and that all medication refills are obtained in a timely manner.
  • Encourages, plans, and assists with daily interactions and weekly face to face visits between child and parent. Ensures appropriate documentation.
  • As the designated Education Specialist: attends IEP and educational planning meetings and arranges for educational placement and transportation for youth.
  • Meets with all youth on a weekly basis.
  • Maintains regular contact with parents and referral sources.
  • Completes discharge planning and report within 10 days of discharge.
  • Direct, supervise and promote the operation of the Shelter for the benefit of the youth.
  • Assess the present program efforts and develop and coordinate a process for measuring program effectiveness and recommend program changes.
  • Meet with Clinical supervisor on a regular basis and Supervisor on a regular basis.
  • Responds to emergency and/or difficult situations during off-duty hours.
  • Coordinates admissions, discharges and daily needs of youth and families with the Shelter Program Coordinator.
  • Monitors and reviews the intake, admission and discharge processes for all diversion and shelter placements.
  • Ensures timely data entry of intakes, admissions and discharges. Completes CWES screening tool as needed.
  • Review all critical incident reports and log into DHS system as required.
  • Maintain all licensure and contract standards. Ensures consistent compliance with all relevant laws, regulations, standards, policies and procedures.
  • Ensure that all required documentation is present in the child’s case record.
  • Develop a method for ongoing self-assessment of licensure and contract compliance
  • Ensures that all data is appropriately collected, documented and reported. To include required DHS reports.
  • Supervise the administration and operation of the School Food program.
  • Develop menus in compliance with DPI requirements which includes review the record keeping of the nutrition specialist for accuracy and compliance.
  • Conduct required training and ensure that these training requirements are met.
  • Keep accurate records.
  • Assume other duties as assigned by the Executive Director

REQUIRED EDUCATION AND EXPERIENCE:

Caseworkers shall possess a bachelor’s degree in human services or a related field from an accredited four-year college recognized by the Council for Higher Education (CHEA) with a minimum of one (1) year of full time experience in Child welfare services or juvenile justice services; or a master’s degree in human services, juvenile justice, or a related field from an accredited college or university with a minimum of one (1) year of full time experience in Child welfare services; or an associate of arts degree in human services or related field from an accredited college or university plus four (4) years of full time experience in Child welfare services.
