What are the responsibilities and job description for the Case Manager Rapid Rehousing Program position at Youth Shelters & Family Services?
Join our team as a dedicated Case Manager, guiding homeless youth and young families through the journey to stable housing and self-sufficiency.
We are currently seeking a dedicated and compassionate individual to join our team as a Case Manager for our Rapid Rehousing Program located in Santa Fe, NM.
What would my job responsibilities be?
- Conduct thorough assessments of the housing barriers faced by homeless youth and young families, tailoring interventions to address their unique housing and service needs while actively assisting them in securing housing options that align with their preferences and circumstances.
- Act as a continual intermediate between property managers and program participants, facilitating effective communication and resolving any concerns or conflicts that may arise, while also fostering positive relationships between participants and their neighbors as appropriate.
- Develop personalized housing procurement, financial management, and self-sufficiency plans in collaboration with clients through thorough intake interviews, ensuring alignment with their individual needs, goals, and eligibility criteria, while also providing discharge and after-care planning and support as needed.
- Conduct proactive follow-up home visits to monitor stability and progress towards self-sufficiency, offering support, advocacy, and problem-solving assistance, while identifying and mobilizing resources to facilitate participants' reintegration into the community and reduce isolation.
- Assist participants in formulating strength-based, solution-focused individualized goal and action plans aimed at achieving permanent housing and self-sufficiency, while establishing and maintaining a timely referral network to provide ongoing guidance and support.
- Conduct regular assessments of participants' life skills needs and provide tailored support and training in areas such as communication skills, problem-solving, conflict resolution, time management, and basic household management.
- Coordinate with local educational institutions, vocational training programs, and employment agencies to facilitate access to education, job training, and employment opportunities for participants.
- Organize and facilitate workshops and group sessions on topics relevant to independent living, such as financial literacy, nutrition, health and wellness, job readiness, and interpersonal skills.
- Collaborate with community partners to develop and implement recreational and social activities that promote community engagement, peer support, and positive social networks among participants.
- Provide crisis intervention and support services to participants facing immediate housing instability, mental health challenges, substance abuse issues, or other emergencies.
- Advocate for participants' needs and rights within housing systems, educational institutions, social service agencies, and other relevant organizations.
What are the qualifications and experience I need to become a Case Manager?
- Demonstrated experience in providing life skills training, vocational guidance, or related services to vulnerable populations, particularly homeless youth, or young adults.
- Familiarity with trauma-informed care principles, motivational interviewing techniques, and evidence-based practices for supporting youth in transition.
- Ability to effectively communicate and collaborate with diverse individuals and groups, including youth from different cultural backgrounds, LGBTQ individuals, and individuals with disabilities.
- Experience in program development, curriculum design, or workshop facilitation is highly desirable.
- Knowledge of community resources and social service systems relevant to housing, education, employment, healthcare, mental health, and substance abuse treatment.
- Strong interpersonal skills, including empathy, active listening, conflict resolution, and boundary setting.
- Flexibility to adapt to changing priorities, work independently with minimal supervision, and collaborate effectively within a multidisciplinary team environment.
What are the minimum education requirements?
- Bachelor Degree in Human Services preferred
- OR a two years degree in Human related services and at least one year related experience.
- OR a high school diploma or GED and at least two years related experience.
What are the scheduled shift you are looking to fill?
- Monday through Friday - 8AM - 5PM
