This position is advertised from the minimum to the mid-point hourly rate. Salary will be determined based on education and experience at the time offer.
**NOTE: Yuma County reserves the right to underfill the Fiduciary (Grade 13 Min: $26.35/hr. - Max: $32.94/hr.) as a Case Manager (Grade 10) until such time as the successful candidate completes all education and training requirements.
NATURE OF WORK
Under limited supervision, performs work of moderate difficulty in client case management activities; this position has department specific responsibilities.
(Illustrative Only) Responsible for auditing incoming and outgoing data for accuracy, completeness and quality, resolves errors and omissions to prevent errors; conducts interviews with clients, families and community members with knowledge regarding the case; responds to client inquiries, either in person or by telephone. Interacts and communicates with family members; advocates for wards when incident arises and is not resolved in a timely and efficient manner; may be required to testify in contested matters; research information to support decision making, locate relatives and heirs; assists with the preparation of the annual Court ordered Guardian Reports, budgets and procurement of insurance; research, evaluate and determines effective methods for the client financial institutions, insurance, legal medical personnel, community agencies and government entitlement programs; supports and enhances the clients’ emotional and physical stability; arranges for physical care and custody of clients incapable of providing for their own welfare and coordinates follow up appointments and transportation; creates and maintains extensive case files; coordinates funeral arrangements for clients, notify family members and makes arrangements to pick-up personal belongings; coordinates referrals to appropriate County departments, agencies and community resources; develops cooperative professional relationships with local agencies significant to the continuum of care for clients; develops and maintains budgets for clients to ensure funding for sustenance and to meet the client’s needs; participates in meetings and case conferences to ensure the delivery of care and services are provided to the client; performs intake, admission assessment, service planning, reassessment, service monitoring and casework; ensures records are in compliance with County, State and Federal requirements; maintains high standards of confidentiality regarding the client and sensitive information; coordinates medical appointments, transportation or any type of services based on the client’s needs; regular and reliable attendance is required; performs other duties as assigned.
High school diploma or GED and four (4) years experience in a mental health, social service or health agency OR an Associates degree in Social Work and two (2) years experience in a mental health agency, social services agency or health agency; OR an equivalent combination of education and experience; must possess and maintain a valid Arizona driver’s license; requires successful completion of a background check.
Knowledge of: County policies and procedures; case management principles and practices; State and Federal laws pertaining to guardianship and case management; applicable laws and regulations relating to estate management.
Skill in: Interviewing patients and prospective clients; collecting, analyzing and evaluating medical, legal and social services information; analyzing problems and identifying solutions and techniques for resolution of issues; assessing and prioritizing multiple tasks, projects and demands; establishing and maintaining effective working relations with co-workers, other County employees and representatives from other county, state and Federal agencies.
Ability to: Communicate effectively verbally and written; read, write, synthesize data and reports; use modern office equipment and computers; lift at least 25 lbs.; bend, walk and stoop; organize and prioritize deadlines; to work independently.
Work Environment
The usual and customary methods of performing the job’s functions require the following physical demands: occasional lifting, carrying, pushing, and/or pulling; some stooping, kneeling, crouching, and/or crawling; and significant fine finger dexterity; generally the job requires 60% sitting, 20% walking and 20% standing; working at a computer monitor for extended periods required; the job is performed under varying extreme weather conditions and may have a high risk of exposure to blood borne pathogens, TB, body fluids, Hepatitis B.
