What are the responsibilities and job description for the Nurse Navigator position at Yuma Regional Medical Center?
The Value Base Care Nurse Navigator provides specialized patient care, education, and coordination of care; collaborates with hospital and ambulatory clinical staff, case managers, social workers, providers, payors, patient, their families and other community resources to facilitate patient access, appropriate care and avoid hospital admissions or unnecessary emergency visits; functioning as a point of contact for patients throughout their treatment, providing education through rapport and relationship building; mentors other staff members and works to problem solve with identified social determinants of health. The VBC Nurse Navigator RN will focus on developing standing orders, to complete preventative/standard screenings outside of visits, developing processes for patient engagement and education on/ in these screenings, and beginning to address patient disease, system navigation based on opportunities for improved outcomes, quality of care and increased population health. The VBC Nurse Educator makes decisions guided by evidence based best practices, as well as policies, procedures, guidelines and standards.
Responsibilities:
1. Collaborates with organization, payor and community resources to provide seamless care coordination and assist with system navigation.
2. Strong interpersonal and emotional intelligence skills necessary to provide effective care coordination with a wide variety of system personnel, primary and specialty clinic staff, community contacts, payors, patients/families and outside agency personnel.
3. Works collaboratively with key stakeholders within Onvida Health, patient, family, and the community to ensure patient needs are met with the goal of preventing unnecessary patient visits, health emergencies or admissions.
4. Drives system-wide policy development of standing orders/protocols to support standard preventative care screenings and care, working with applicable organizational leaders, medical staff, etc. to ensure alignment to proper procedures and staff.
5. Engages patients with advanced disease, high utilization of services, in additional support and care opportunities
6. Educates patient, through established rapport, regarding disease process, medications, prevention, proper escalation and resources available to support, etc.
7. Other duties as assigned.
Credentials:
* REGISTERED NURSE (RN)
* BASIC LIFE SUPPORT (BLS)
Education:
* BACHELOR'S DEGREE
Minimum Requirements:
· Bachelor's Degree in Nursing or other related field of study
· Current RN Licensure from Arizona State Board of Nursing
· Two (2) or more years related experience
· BLS - Basic Life Support
Preferred:
· Proficient in Epic electronic medical record
· Proficient in Microsoft suite (Word, Excel)
· Experience with value-based care, population health, care coordination, patient education, etc.
· Bilingual (English and Spanish)
