Summary
The Patient Care Assistant II is a member of the healthcare team who provides direct and indirect patient care duties, supporting nursing within the Licensed Nursing Assistant (LNA) scope of practice.
Responsibilities
Adheres to workflows and patient care tasks outlined by training and orientation. Accepts delegation from nurses to provide care in alignment with scope of practice.
Conducts effective handoff between care team members, including communication with the nurse regarding the patient’s care plan each shift. Contributes to the admission, transfer, and discharge processes.
Attentive to patient and visitor needs, maintaining compassionate care that addresses both physical and emotional aspects of well-being and communicates clearly and effectively through established systems concerning relevant information (verbally, non-verbally, and in writing).
Assists patients with activities of daily living (ADL’s), including patient hygiene, pressure injury prevention, and safe mobility and reduces risk of harm to patients through safety interventions.
Serves to transport patient specimens safely and effectively, documents accurately and timely in the Medical Record and appropriately reports to the nursing staff any concerns identified.
Contributes to seamless department operations by demonstrating sufficient work habits such as dependability, workplace cleanliness and organization, supply management and vigilant patient care equipment maintenance and management.
Maintains professional responsibilities related to education, training, and accountability to department communication methods.
May be oriented to unit secretarial duties to include responding to incoming calls, supporting personnel through interdepartmental and interprofessional communications; uses organizational communication systems to notify of codes (e.g. Code White, Code Neuro).
Specific to inpatient locations: purposeful rounding in alignment with Policy, educates patient to call light use, and is committed to call light responsiveness.
Specific to Women’s and Children’s: responsibilities related to registration, certification, patient admission, insurance authorization process, electronic birth records, paternity testing, and recording fetal death.
Other duties as assigned by department leadership.
