Description
The Family Resource Center Case Manager will provide career guidance and case management services for individuals transitioning to self-sufficiency in a way that provides exceptional client and community services while achieving the highest standards of compliance.
Essential Functions

Provide compassionate, empowering assistance to all individuals in a manner that prepares them to achieve the very best in their lives.

Complete thorough assessment of individuals’ progress toward self-sufficiency, training, education, and other workforce development activities. Develop individual employment plan to determine realistic short- and long-term self-sufficiency goals.

Work with DHR’s A-RESET program and as necessary assist individuals with gaining certification through DHR.

Provide case management to active SNAP E&T participants and up to 90 days of follow-up case management to those clients who secure employment, track their stabilization and provide support as needed throughout follow-up period.

Provide individualized case management services and advocacy to assist individual’s with identifying and overcoming barriers, enrolling in job training, obtaining employment, as well as referrals to outside agencies (medical, food stamps, employment, educational, financial, legal, mental health issues and/or others) and other voluntary services that are available to them and that might best serve their particular needs and aspirational goals.

Determine client’s needs for financial assistance and complete appropriate referrals to support their tasks related to job training and employment goals.

Build relationships and connect with residents to help identify, initiate, and execute services and planned events in a manner consistent with the focus on health, well-being, education and cultural expression.

Maintain thorough information about local resources and programs including but not limited to job training, job placement, workforce development, and educational.

Assist with identifying and recruiting participants for FRC’s workforce development program and services.

Assist with all audit, funding or educational visits to the YWCA through public speaking and touring of the facilities as may be required including representing the YWCA at meetings and community events as may be requested.

Maintain applicable client files, case notes, and detailed records to ensure consistency and compliance with all agency procedures, programs and/or funding streams, doing so in concert with the guidance of the YWoodlawn Family Resource Center.
Position Type and Expected Hours of Work
This is a full time position. Days and hours of work are Monday through Friday 8:00 a.m. to 5 p.m.
Required Education and Experience

Bachelor’s Degree from an accredited college or university

3 plus years of experience providing direct services to individuals, children, and families.

Computer skills in word processing and a willingness to adapt to new software as may be needed.

Valid driver’s license and personal automobile insurance if applicable to position.
