COMMUNITY SUCCESS NAVIGATOR

STATUS : Full-time

Rate of pay : $23.03 per hour

Summary :

The Community Success Navigator is a compassionate and dedicated individual with a passion for building community and empowering people to thrive. The ideal candidate has experience connecting families to valuable resources, demonstrating exceptional interpersonal skills, empathy, and a commitment to fostering growth and well-being. Navigator acts as a conduit and advocate to guide individuals across different YWCA programs and services, and those of partner agencies and organizations. By providing comprehensive support for a family’s multiple needs, we believe individuals and families gain the tools and resources to overcome challenges and achieve long-term self-sufficiency and improved outcomes.

Responsible for the implementation of the YWCA Integrated Service Model, which seeks to address multiple Social Determinants of Health (SDOH) including homelessness and housing needs, food insecurity, physical and mental health, legal support, education and training, workforce development, affecting the quality of life for all YWCA women and families. The Navigator will be responsible for the implementation and coordination of essential service delivery by maintaining an ongoing relationship with participants.

Essential Duties and Responsibilities include the following :

  • Conducts initial intake to assess the needs of women and their families, including a needs assessment to gauge needs across the Social Determinants of Health (SDOH) and success, using a trauma-informed approach.
  • Develops trust-based relationships with participants and maintains ongoing connection to ensure participant’s continued success.
  • Educates participants about YWCA programs and resources, and facilitates referrals to community partners.
  • Provides coordination support to navigate participants through YWCA and external resources to address and resolve identified needs. With participant, creates a series of goals that informs the creation of a support plan.
  • Makes warm introductions to internal and external programs to meet immediate participant needs and stabilize the participants situation.
  • Coordinates with local social service agencies, county and state resources, and other community partners addressing Social Determinants of Health (SDOH) and success factors affecting the participant.
  • Documents service delivery and participant progress, keeping detailed records and data to inform service delivery and program development.
  • Follows up with participants at determined frequencies to monitor and support participant progress toward goals; identifies new or emergent needs and updates support plans.
  • Attends community events on behalf of YWCA to reach new participants and share information about our programs and services.
  • Performs other duties as assigned.

Requirements : Qualifications :

Education / Experience :

Bachelor’s Degree in Social Work Human / Family Services, Counseling, Psychology or related field with two (2) to three (3) years of related experience. Thorough knowledge of principles of social work, crisis intervention, counseling, or psychology which is typically gained through experience in employment, practicum and / or volunteering required, and related experience preferably in a related non-profit environment or equivalent combination of education and experience. Knowledge of the impacts of the social determinants of health required. Considerable knowledge of intake methods and techniques and case management procedures are necessary.

Communication Skills :

Ability to communicate effectively and empathetically, both orally and in writing, with diverse population and groups. Ability to build a rapport with participants that are facing many challenges at once. Ability to stay calm and empathize with program participants when they are upset. Ability and willingness to maintain strict client confidentiality.

Critical Thinking Skills :

Ability to give full attention to what participants share, understand the points being made, ask questions, and respond appropriately. Thorough knowledge of all programs, principles and practices at YWCA South Florida and community partners to facilitate optimal service delivery to improve the lives of participants.

Resource Navigation Skills :

Knowledge of the impact of trauma and poverty on the local population. Ability to develop and implement short and long-range plans to meet program and participant needs. Knowledge of federal, state, and local policies, procedures, and regulations applicable to program areas. Ability to establish and maintain effective working relationship with agency staff, gatekeepers and general public.

Language Ability :

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Bilingual skills in Spanish and Creole preferred.

Reasoning Ability :

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to analyze and investigate requests from the public and coordinate effective service delivery.

Computer Skills :

To perform this job successfully, an individual should have a working knowledge of Microsoft Office applications to include Word, Excel, and PowerPoint. Data management experience is mandatory.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
