Housing Navigator

We are hiring a full-time Housing Navigator to provide case management services to participants in our Housing Programs, including intake, screening, referrals, and monitoring. This position ensures that participants have goals, written plans, and the support to achieve the goals. The Housing Navigator builds and maintains community relationships to ensure effective coordination of services.
The pay range for this position is $22.00-$24.00/ hour.
In this role, you will:
· Provide case management services to housing program participants, including intake and screening, referrals and monitoring, which includes following-up with transitional housing program participants 6 months post-exit.
· Work with participants to identify goals and barriers and develop written plans.
· Serve as an advocate and referral source for participants in need of domestic violence screening and assessment, counseling, and crisis services.
· Facilitate new resident orientations, life skills programming sessions, and other resident meetings.
· Provide crisis case management and de-escalation.
· Maintain confidential case files, ensuring timely and accurate documentation.
· Build and maintain community relationships to ensure effective coordination of services.
Minimum Qualifications:
· Bachelor’s degree in social work, human services, or related field.
· Minimum of two years of experience in case management.
· Commitment and ability to work with diverse populations.
· Strong interpersonal communication skills and the ability to manage conflict constructively.
· Ability to maintain confidentiality.
· Skilled in managing time and meeting deadlines.
· Basic ability in Microsoft Word, Excel, and Outlook.
· Valid driver’s license and access to a safe, reliable and insured vehicle.
Preferred Qualifications:
Experience working in the non-profit sector.
Bilingual (Spanish, Hmong, Karen or Somali)
