What are the responsibilities and job description for the Housing Case Manager position at YWCA Western New York?
The Housing Case Manager is responsible for providing case management services and case coordination for our clients at School House Commons. This includes leading social, wellness, and recreation programming, and ensuring that the unique needs of our diverse residential community are met. Developing partnerships with community-based organizations and community stakeholders is an integral component of successful housing programming. This person must have or be willing to develop working knowledge of working with the senior population, homeless issues, client advocacy, community resources, drug and alcohol issues, and mental health issues. Exercising discretion is imperative, and maintaining the confidentiality of all case information is a requirement of the role.
The Housing Case Manager is a dynamic, reliable person who can work independently as well as part of a team in a fast-paced, evolving environment. Exceptional communication and organizational skills are essential.
The position is full-time, 40 hours per week with occasional evening and weekend commitments.

Required Skills and Abilities
Case Management
- Complete intake assessment for each client upon move-in.
- Develop and complete Independent Development Plans, based on the individual client’s needs.
- Complete discharge documentation upon move-out.
- Provide routine case management to senior residents with empathy and urgency to client needs, documenting all interactions.
- Ability to work effectively with diverse populations; ability to overcome language barriers.
- Must have the ability to establish strong working relationships with clients and colleagues, maintain professional boundaries with clients, and demonstrate professional ethics.
- Develop collaborative relationships with community-based organizations to facilitate programming and to meet client needs as identified via case management.
- Maintain documentation and data as required by funders, both physically and electronically.
- Provide intervention and take appropriate action during crisis and emergencies.
- Communicate the necessary information to leadership as needed.
- Attend meetings and/or training as required; and
- Stay current with research and literature related to emerging trends and promising practices within supportive housing for seniors
Coordinated Activities
- Schedule and provide programs and events, based on client interest and social, emotional, and wellness needs, which encourage engagement and socialization to help support a high quality of life
- Purchase and prepare materials needed for activities; set up and clean up activity spaces
- Create and distribute monthly newsletters, calendar, and regular activity flyers
- Prepare social media posts including photographing activities and writing captions, may also appear in social media posts in support of the YWCA and its programs.
- Coordinate transportation of residents to and from YWCA WNY- sponsored activities and events held off-site
- Ensure activities meet grant objectives
Qualifications
- Bachelor’s degree in Social Work is preferred, or related field with 5 years’ case management human services experience.
- Extensive experience in housing management, preferably within affordable housing or supportive services
- Candidates must possess strong organizational abilities and the ability to multitask in a fast-paced environment.
- Excellent oral and written communication abilities and interpersonal skills are required.
- Proficient in Microsoft Suite
- Dependable and flexible, with ability to quickly respond to changing priorities.
- Ability to be productive in high-demand position.
- Must possess a valid driver’s license.

Pay: $43,000.00 - $48,000.00 per year
