Your role will be to address their needs and provide valuable feedback to help us continually improve our services.
Role Description
Zócalo Health is seeking an organized and compassionate Intake Specialist to serve as the initial point of contact for patients accessing our care services. This role is essential in ensuring that patients receive seamless support as they begin their healthcare journey with Zócalo Health. The Intake Specialist will complete intake appointments, manage patient documentation, and coordinate follow-up care. This role also supports outreach events and documentation for outreach and claims submission efforts.
The ideal candidate is bilingual in English and Spanish, highly organized, detail-oriented, and passionate about improving access to healthcare in underserved communities.
About the Role
Patient Intake and Coordination
- Complete scheduled intake appointments with patients to collect necessary information and consents. Intake appointments can be virtually or in person.
- Update and maintain the county-based case management tracker for all patients.
- Create care planning accounts for new patients.
- Schedule follow-up appointments with the assigned case manager and/or the Zócalo Health clinical team.
Outreach and Community Engagement
- Attend outreach events and professionally represent Zócalo Health to community members and partners.
- Collect and document patient interest and contact information at community events.
- Support outreach efforts by assisting with Athena documentation for outreach attempts and claim submission.
Documentation and Communication
- Accurately document patient information and activities in electronic health records (EHR) and trackers.
- Escalate any issues or barriers to care to the appropriate team members promptly.
- Collaborate with members of theClinical Operations teams to ensure patients receive timely and coordinated care.
Requirements for the Role
Education and Experience
- Minimum of 1-3 years of experience in healthcare, customer service, or related fields.
- Previous experience with patient intake, scheduling, or outreach preferred.
Skills and Competencies
- Fluent in English and Spanish (verbal and written).
- Exceptional interpersonal and communication skills to build trust and rapport with patients.
- Detail-oriented with strong organizational skills to manage multiple tasks simultaneously.
- Comfortable with technology and proficient in using electronic health record systems (Athena experience preferred).
