Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges. This role involves assessing client needs, developing treatment plans, and connecting clients with appropriate resources. The Social Worker will work collaboratively with other professionals to ensure the best outcomes for clients, particularly in areas such as addiction counseling, geriatrics, and special education.
Responsibilities

- Consistently contributes to the organization’s vision and mission statements and demonstrates the Values of the organization.
- Ensures the health and safety of program participants using a proactive approach.
- Provides case management services for program participants living in a group home setting.
- Maintain a caseload of up to 6 program participants.
- Contributes to the timely completion and necessary revisions of Needs and Service Plans
- Develop and participate in Child and Family Teams.
- Aid in case planning and consultation with team members on clinical and casework issues, as appropriate.
- Participates in Emergency Intervention Plan (EIP).
- Ensures the rights of program participants and staff are respected.
- Provides (EPSDT) Medi-Cal Specialty Mental Health Services (SMHS), including individual and group rehabilitation, targeted case management, intensive care coordination, and others.
- Completes all documentation per Medi-Cal, agency, county, and state requirements.
- Develops an emotional bond with the participant and helps participants succeed in life to meet new goals.
- It helps program participants make good decisions about their current situation and future.
- Supports both program participants and caregivers in identifying and accessing services and supports to heal from trauma, reduce the risk of re-traumatization, and foster well-being and resiliency.
- Develops coping, communication, and self-regulation skills in program participants.
- Develop independent living skills that foster self-esteem, self-advocacy, and empowerment.
- Focuses on conflict resolution and high school completion.
- Encourages program participants to stay in school, assists with homework, and helps cultivate academic skills.
- Able to recognize and understand the trauma that children have experienced and how trauma impacts children.
- Identifies behaviors and evaluates to assess the need for services.
- Implements Needs and Service plan.
- Provides activities that require group interaction.
- Gets program participants involved in beneficial activities.
- Gives program participants a safe and supervised environment to spend their free time.
- Assesses program participants and gathers relevant information.
- Offers information supporting program participants and their families.
- Contacts and makes referrals to other agencies and services.
- Assists program participants in receiving services.
- Maintains accurate records and prepares reports.
- Participates in training, supervision, and meetings.
- Attends meetings with program participants, residential staff, and administration as required.
- Adheres to all federal, state, and local rules including company policies and procedures.
- Provides verbal and non-verbal crisis de-escalation and intervention for program participants.
- Conducts treatment plans, crisis management assessments, risk management assessments and soap notes for participants.
- Any other duties that may be necessary to ensure the health and safety of children in the Facilities. Skills
- Strong knowledge of social work principles and practices.
- Proficiency in addiction counseling techniques.
- Familiarity with HIPAA regulations regarding client confidentiality.
- Experience in data collection and analysis for program evaluation.
- Understanding of geriatrics and the unique needs of older adults.
- Research skills to contribute to evidence-based practices in social work.
- Knowledge of psychopharmacology as it relates to mental health treatment.
- Ability to work effectively within special education frameworks.
- Excellent case management skills, including organization and communication abilities.
