MDS Nurse

Position title: MDS Nurse

Requirements:

1. Must be a qualified Registered Nurse or Licensed Practical Nurse, licensed to practice nursing in Oklahoma.

2. Must be physically and mentally capable of performing the duties required.

3. Should have at least three years of experience in a hospital or long-term care facility and have training in rehabilitation or geriatric care.

4. Knowledge of Medicare billing procedures and MDS documentation.

5. Ability to work in a team environment.

6. Must be able to lift a minimum of 50 pounds.

7. Must be willing to work some weekends and some holidays.

Supervisory Controls:

Reports to the Administrator of Health Services.

Responsibilities and Duties:

1. Develops, updates, and maintains care plans according to state and federal regulations.

2. Develops and maintains a good rapport with all services involved with the care plan team to ensure that a team effort is achieved in developing the resident’s comprehensive care plan. Will make necessary adjustments to care plans as needed and will partner with the care plan team to identify significant changes.

3. Collects and analyzes patient data to ensure accurate and complete medical documentation.

4. Provides education and training to staff on proper documentation practices.

5. Coordinates and Completes the RAI Process for the MDS according to facility policy, state and federal regulations. Assures signatures are completed as required.

6. Develops and maintains the MDS schedule and works with the interdisciplinary team to determine appropriate timeframes for assessments and completes assessments on residents. Assures Assessment Reference Date accurately captures patients' needs and treatments to ensure accurate payment.

7. Participates in care plan meetings, daily meetings, community training, and other meetings as needed.

The following duties may be included in the work schedule for this position:

Giving and receiving the daily reports, implementing emergency procedures; implementing nursing care plans; utilizing of medical records system, and other duties outlined by Zarrow Pointes’ Health Care Center’s policies and procedures manual.
