What are the responsibilities and job description for the CCP SURERVISOR position at Zayan LLC d/b/a ActiveCare Home Care?
CCP SUPERVISOR:
A Home Care Supervisor will manage/coordinate home care services, ensure compliance with policies and procedures, and provide leadership to the caregiving staff.This position oversees HCA/FHCA who provide and support our client’s personal care. Supervisors are responsible for scheduling Home care Aid’s visitations and work assignments and ensuring the quality of care that is provided is up to the agency standards. We are currently seeking a highly organized and experienced Home Care Supervisor to join our team at our Palos hills branch office.
Duties include but not limited to:
- Identifies and assigns homemakers to meet client care plans.
- Maintains accurate records and service logs.
- Ensures Electronic Visit Verification (EVV) compliance
- Ensuring that high-quality care is provided to all home care clients
- Documenting care plan progress and outcomes to improve care in the future
- Overseeing the day-to-day operations of a home health agency
- Ensuring that staff is well trained, that patients are receiving quality care, and that all company policies and procedures are being followed.
- Address and resolve client or family concerns promptly and professionally.
- Maintain accurate records and documentation of care services.
- Monitor caregiver performance and provide feedback for improvement.
- Communicates with homemakers routinely and as needed in addition to required supervisory home visits about, but not limited to: issues with the quality of work, changes in client needs, technical issues, etc
- Communicates with the Case Management Agency about the services that are needed for each client and changes in clients.
Requirements:
- Highschool diploma and 2 years work experience in related fields
- Driver’s license and reliable transportation
- Strong communication and organization skills
- Ability to multitask and handle a fast-paced work environment
Job Type: Full-time
Pay: From $42,000.00 per year
