GENERAL SUMMARY:
Responsible for processing the patient paper medical record and converting it into an Electronic Medical Record (EMR) in a timely manner for the viewing of physicians, clinicians, and other designated health care employees for the treatment and care of the patient.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
- Ensures all pages have the appropriate patient identification and form identification barcode.
- Reviews the paper record against the electronic document to verify image quality and missing documents.
- Verifies and validates image/document errors and makes necessary changes as needed.
- Accurately identifies destination of record and/or loose report.
- Adheres to the defined schedule and expectations as determined by HIM Management.
- Communicates any record discrepancies or irregularities to HIM Management.
- Maintains productivity and quality levels in accordance with current standards.
- Performs other duties as assigned or required.
- On-Campus Scanning
- Performs near-time scanning of all active and discharge patient records on nursing floors.
- Retrieves and scans discharged records from all units and retrieves from outpatient and ED's and prepares for transport
- Process all loose documents in a timely manner.
- Notifies Health Information Management (HIM) Document Imaging (DI) Lead if supplies are needed or low in the DI storage room. Monitors, maintains, and re-stocks supply levels on WOW
- Utilizes designated discharge log on units to verity receipt of all records.
- Communicates effectively with nursing and ancillary staff regarding inquiries on the location of records and/or missing
- records.
KNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED:
- Must have healthcare experience.
- Experience with Health Records is a plus.
- Must have Epic experience or experience working with other hospital systems like Cerner/PowerCharts/Meditech
- Intermediate computer skills including, but not limited to, Microsoft Office Word, Outlook, and Excel required.
- Excellent interpersonal and communication skills required.
- Critical thinking and detailed work skills required.
- Ability to perform repetitive tasks and maintain focus required.
- Ability to retain information and maintain a high level of accuracy required.
- Will be assigned a Walking Work Station. Must be able to stand/walk for extended periods of time.
KNOWLEDGE AND EXPERIENCE PREFERRED:
- Previous scanning and clerical experience preferred.
- Knowledge of medical terminology preferred.
