Registered Nurse (RN) Case Manager
Location:- Medford, OR 97504
Duration:- 13 Weeks
Shift:- Day 5x8-Hour (08:00 - 17:00)
Pay rate:- $60 - $65/hr
Responsibilities of Registered Nurse (RN) Case Manager
- Collaborate with interdisciplinary healthcare team members to assess, plan, implement, coordinate, monitor, and evaluate the care of patients with complex medical needs.
- Coordinate patient care services, including home health services, durable medical equipment, and community resources, to facilitate safe transitions of care and promote continuity of care.
- Monitor patients' progress toward goals, evaluate outcomes, and adjust care plans as needed to optimize patient outcomes and resource utilization.
Requirements for Registered Nurse (RN) Case Manager
- 1 years of experience.
- RN License.
- BLS.
