JOB DESCRIPTION
POSITION TITLE: Customized Community Support-Group Provider
REPORTS TO: Direct Services Manager
POSITIONS SUPERVISED: None
SUMMARY: Provide direct care services to individual(s) according to local, state and federal standards.
Must be at least 21 yrs. Old
Must of been licensed for driving for at least 3yrs.
ESSENTIAL FUNCTIONS:
1. Provide direct care services to DDW and SGF individuals of all ages and levels of need.
2. Maintain the health and safety of one or more individuals at one time, while providing services either in group or individual settings.
3. Advocate for and support all individuals served and their families in accordance with service standards and regulations.
4. Assist the individual’s efforts to engage in meaningful and positive interactions with others.
5. Provide individuals the opportunity for progress toward outcomes and action steps in accordance with their ISP (Individual Service Plan).
6. Assist all individual’s, as needed, in working toward their desired outcome goals.
7. Support strategies implemented in individual’s specific therapy plan(s) and/or behavioral support plan.
8. Demonstrate participation and initiative in all of the program’s daily activities/services.
9. Communicate progress of services or barriers to the provision of services with Service Coordinator(s) or management.
10. Complete required forms, reports and documentation in a timely manner and in accordance with service standards and regulations. This includes: daily encounter logs, mean and attendance counts, pre-ISP planning worksheets, General Event Reports and ANE Reports.
11. Attend individual’s service planning meetings as requested.
12. Provide or assist individuals being served with transportation needs. This includes: knowledge in completing the daily choice Board and required transportation trainings.
Transportation trainings include: Customer Service, Sensitivity and Assistance; Defensive Driving, Crisis Management and pre & post trips process and reports.
13. Participate in professional meetings as required.
14. Complete and maintain training requirements per service standards and regulations. Which includes: all 12 DDW core classes, CPR-First Aide, Individual Specifics, and annual renewals as required.
15. Display ethical behavior at all times.
16. Maintain confidentiality of individuals, families, and coworkers.
17. Display a professional and positive attitude towards individuals and their caregivers.
18. Display a professional and positive attitude towards coworkers and supervisors.
19. Remain compliant with applicable service standards and regulations governing services provided. Which includes training on the DDW standards pertaining to the group setting.
20. Remain compliant with Zia Therapy Center, Inc. policies and procedures.
21. Observe all appropriate safety guidelines.
22. Other related duties as assigned.

24. Work with and provide assistance to other staff while working within group setting.
NON-ESSENTIAL FUNCTIONS: None
EDUCATION AND/OR TRAINING REQUIRED: High school diploma/GED required.
WORK EXPERIENCE PREFERRED: Working effectively individually or in a fast-paced team environment.
