Position Description:
Part-Time Social Worker (SSW, BSW, MSW, LCSW) working in Cedar City, UT providing direct patient care to home health and hospice clients. Current social work license required. Assess the psychosocial status of patients and families/caregivers; carries out evaluations and plans interventions and resources based on findings; educates and counsels as needed in relationship to stress and coping; maintains clinical records on patients; provides information and referral services for patients and families/caregivers regarding practical and environmental needs; participates in the development of the individualized plan of care and attends regular interdisciplinary team meetings; etc. Collaboration and communication with other team members and disciplines. Work to help build and maintain essential volunteer base. Offer the needed bereavement support to patients and families as they face declining health, dying, and death of loved ones.
