Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring effective discharge planning, and managing medical records to facilitate optimal patient outcomes. The ideal candidate will have experience in hospital medicine, ICU, or primary care settings and possess strong skills in patient care and care coordination.
Facility Requirements:
- Must have BLS certification required, (Must be through the American Heart Association); RQI certifications will not be accepted
- Must have 2 years RN Case Management experience, with experience in a long-term acute care setting. discharge planning, clinical case management & utilization review
- Must have Kentucky RN license.
- Shifts are Monday - Friday 9a-5p; no on-call or weekends
Duties
- Assess and evaluate patient needs to develop comprehensive care plans.
- Coordinate with multidisciplinary teams to ensure continuity of care throughout the patient's hospital stay and post-discharge.
- Manage medical records accurately and maintain confidentiality in compliance with healthcare regulations.
- Facilitate discharge planning by collaborating with patients, families, and healthcare providers to ensure a smooth transition from hospital to home or other care settings.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Provide education and support to patients and their families regarding treatment options, resources, and follow-up care.
- Participate in quality improvement initiatives to enhance patient outcomes and streamline case management processes.
Experience
- Current Registered Nurse (RN) license in good standing.
- Previous experience in a hospital setting preferred, particularly in ICU or primary care environments.
- Knowledge of medical records management and discharge planning processes is essential.
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Ability to work collaboratively within a team-oriented environment while managing multiple priorities efficiently.
