Duties
- Provide comprehensive care management services to individuals in need

- Coordinate and monitor services to ensure effective delivery of care
- Collaborate with healthcare providers, social services agencies, and community resources
- Implement crisis intervention strategies when necessary
- Utilize motivational interviewing techniques to support clients in achieving their goals
- Oversee childcare services and provide supervision as needed
Experience
- Previous experience in social work, child welfare, or related fields
- Knowledge of addiction counseling and Medicare regulations
