Description
Our team is seeking a part-time or full-time Adult Case Manager to provide services to clients. As an Adult Case Manager you will help clients gain access to needed medical, social, educational, vocational, and other services as they relate to mental health needs. Case management services includes developing a functional assessment and integrated treatment plan, referring and assisting clients to obtain needed services, ensuring the coordination of services, and monitoring the delivery of services.

Responsibilities
- Maintain a caseload of clients.
- Coordinate and make referrals for client services that will help the individual meet their goals.
- Complete and accurately update all documentation as required.
- Work collaboratively with all members of the treatment plan as often as needed.
- Follow-up on referrals and monitor the clients progress.
- Provides crisis assessment and intervention services to clients as needed.
Qualifications
Requirements & Qualifications
- Bachelor’s Degree in Social Work, Psychology or a related human services field.
- One year of experience providing services to individuals with a mental illness or substance use disorder required.
- Must qualify as a Mental Health Practitioner in the State of MN.
- Licensed as a Social Worker in the State of MN required if graduated with Bachelor's of Social Work.
- Provisional License in the State of MN will be considered.
- Experience working with severe emotional disturbances in adults is preferred.
- Valid driver's license and proof of auto insurance required.

Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.
