Zumbro Valley Health Center is seeking a full-time Adult Case Manager to provide exceptional care to clients that have been diagnosed with a mental illness. Our team works hard to help clients gain access to services needed and support them through their recovery.
Responsibilities
- Maintain a caseload of clients.
- Coordinate and make referrals for client services that will help the individual meet their goals.
- Complete and accurately update all documentation as required.
- Work collaboratively with all members of the treatment plan as often as needed.
- Follow-up on referrals and monitor the clients progress.
- Provides crisis assessment and intervention services to clients as needed.
Qualifications
Requirements & Qualifications
- Bachelors Degree in Social Work, Psychology or a related human services field.
- 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
- Must qualify as a Mental Health Practitioner in the State of MN
- Licensed as a Social Worker in the State of MN preferred.
- Provisional License in the State of MN will be considered.
- Experience working with severe emotional disturbances in adults is preferred.
- Valid driver's license and proof of auto insurance.
Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.
